Try-out Information
Athlete Checklist
Rochester Community Schools utilizes Final Forms for collection of physical forms. If you have a High School athlete, click on https://rochester-mi.finalforms.com/ to set up an account and get your student-athlete registered for the upcoming season.
Athletes must be fully registered through Final Forms and have their up to date sports physical uploaded into the site. Athletes will not be allowed to tryout or practice until they are completely registered into final forms.
Pay to participate fees can be paid online through Eleyo. Payments should be made before the first athletic contest, please upload the payments receipt into final forms after payment has been made.
